A Silver color metal and enamel device 1 1/8 inches (2.86 cm) in width consisting of a lozenge-shaped field with scalloped edges, quartered Azure and Maroon with a Silver pine tree at its center, and overall a Red cross charged at center with a Silver five-pointed star, the whole encircled by a Green scroll open at the top of the device, with the words "HEATH CARE IN RESERVE" in Silver letters.

The medical mission of the organization is referred to by the red cross and the use of silver and maroon, colors usually associated with the Medical Corps.  Maine, the Pine Tree State and home of the hospital, is represented by the pine tree.  The state flag is referred to by the blue quarters and the star.  Green alludes to the unit's mission of healing and is further reference to the trees of Maine and its timber industry.

The distinctive unit insignia was approved on 24 February 1986.
